Passport Renewal - Document Checklist

Please gather the documents below and upload clear, full-page scans in PDF Format to the secure Google Drive folder / Form link we provided.

Mandatory Documents

  1. Old Passport
  2. Aadhaar Card ( E-Aadhaar with a verifiable QR code)
  3. School Leaving Certificate
  4. 10 / 12 / Bachelors / Last education Mark-sheet + Certificate
  5. Marriage Certificate ( If Married )

These are mandatory documents, without it we cannot move forward with the process

Proof of Address ( Any Two )

  1. Aadhaar Card (E-Aadhaar with a verifiable QR code)
  2. Voter ID Card (EPIC)
  3. Electricity Bill (latest copy, not older than 3 months)
  4. Photo Passbook of your active Bank Account
  5. Driving License

This document must show your current residential address.

Proof of Date of Birth ( Any Two )

  1. Birth Certificate
  2. PAN Card
  3. Driving License

This document must clearly state your full name and date of birth.

Notes :

  1. Document Quality: Please ensure your scans are not blurry, cut-off, or dark. We cannot use photos of laminated documents.
  2. Address Proof Must Be in Your Name: Utility bills (electricity, water, gas) must be in the applicant's own name. If they are in a parent's or landlord's name, they are not valid. In that case, a Registered Rent Agreement in your name is the best alternative.
  3. Originals are Mandatory: You must carry all your original documents (the same ones you uploaded) to your Passport Seva Kendra (PSK) appointment.

If the name on the marriage certificate does not match the other documents, a Joint Declaration (referred to as 'Annexure J') is required.

To prepare this declaration, we will need:

  • The wife's full maiden name (formatted as: Surname, First Name, Father's Name).
  • The wife's Aadhaar Number.
  • One (1) joint photograph of the couple (PAN card size).
